Everything had changed, but nothing had changed at all.

The gardens that used to bloom with roses were now overgrown and dead.

Only the chapel remained—grand, ornate, and completely out of place.

Inside the main hall, Isabella sat in a wheelchair, her face pale.

When she saw me walk in behind Luca, hatred burned in her eyes.

But in front of Luca, she put on a weak, pitiful act.

“Luca, is this the doctor?”

“She looks just like—”

“Shut up.”

Luca cut her off, his voice cold.

“This is Dr. Alessia. She’s here to save you.”

“Do what she says. Get on your knees and beg.”

Isabella’s eyes went wide with disbelief. “What? You want me to kneel in front of her?!”

“She’s that bitch Elena! I won’t do it!”

She tried to stand up, but Luca’s glare made her shrink back.

“Isabella, do you want to live?” Luca’s voice was soft—but laced with the threat of death. “Or should I just stand here and watch you die?”

Isabella shook.

She knew Luca wasn’t the same man anymore.

Five years of guilt had worn down whatever patience he’d had left for her.

If it weren’t for the so-called “debts of the past,” he would have thrown her out long ago.

Isabella bit her lip and slid off the wheelchair.

Her knees hit the marble floor hard.

“Dr. Alessia… please… save me.”

I sat on the velvet sofa, looking down at her.

I’d waited five years for this moment.

Back then, she’d watched them drag me to the operating table.

Now, the tables had turned.

“Louder. I can’t hear you.” I picked at my ear lazily.

Isabella trembled with rage, her nails digging into the carpet.

“Save me!” she shrieked.

I nodded in satisfaction and stood up.

“Good.”

I pulled out a set of needles. The silver tips glittered under the chandelier.

“Since Miss Rossi is so sincere, let’s begin the first treatment.”

“It might sting a little. Try to bear it.”

Before she could react, I drove a needle into her.

“AHHH—”

Her scream echoed through the entire estate.

Luca stood nearby, watching coldly.

Even as Isabella writhed in pain, his expression didn’t change.

I saw it, and all I felt was irony.

This was a man’s love.

Once, he’d sacrificed me to save her.

Now, to atone for my death, he watched her suffer.

When the treatment was over, Isabella passed out from the pain.

I packed up my needles slowly, then looked at Luca. “Don, something on your mind?”

Luca was quiet for a moment. Then he nodded.

We went into the study. I locked the door behind us.

The air turned suffocating.

“Elena. Stop pretending.” Luca’s voice came out rough. “I know it’s you.”
