The prenatal vitamins my adopted sister, Gianna Lazzari, had given me tore through my stomach, a deep, twisting cramp that wouldn’t let up.

My eldest brother, Alessio Lazzari—the best surgeon the Lazzari family had—said I needed an emergency C-section.

My mafia husband, Rafe Bellandi, lost his mind. “No! Just keep Nina alive!”

Even through the worst of the pain, I felt lucky. In a world as brutal as ours, every person I loved was right there with me.

Then, while I lay there waiting for the anesthesia to kick in, I heard them talking outside the door.

There was an edge to Alessio’s voice. Something almost like excitement.

“Nina’s healthy. Her blood type matches, her age is a perfect match, everything lines up. We’re not gonna get a chance like this again. We should transplant her heart into Gianna.”

Matteo, the Don, the head of the entire Lazzari family, didn’t even hesitate.

“I’ll wipe the records. Nina won’t know a thing.”

Dario, our genius pharmacology professor and one of the best in New York, backed them up.

“I’ll develop something, a custom anti-rejection cocktail to keep her body from rejecting the replacement heart. She won’t feel a thing.”

A long silence. Then Rafe’s voice. Almost heavy with a sick kind of relief.

“Gianna needs a strong heart if she’s ever gonna dance again. Nina… we’ll make it up to her. We will.” He exhaled. “I’ll sign the consent form.”

My whole body went cold.

My chest ached like they’d already cut it open, like the heart was already gone…

That’s when I heard it, a voice I almost recognized, cutting through the haze.

[Nina! You’re about to die! Are you really going to throw away everything, every reward, just to stay in this world?]

I laughed. Or maybe I cried. Maybe both at once.

Not this time. Their twisted version of love wasn’t enough to hold me here anymore.

I fought to roll over, to drag myself off the bed, but a nurse was already on top of me, forcing a breathing mask over my face and an IV needle into my arm.

It happened fast.

The anesthesia hit my bloodstream like a freight train.

But right before everything went black, I saw them wheel Gianna in. The adopted sister they all spoiled rotten. The one who had poisoned me and killed my baby.

Alessio had the scalpel ready. His voice was ice when he turned to the nurse.

“Leave the fetus. Take Nina’s heart first. Gianna’s transplant is the priority.”

The scalpel bit into my chest. I swore I felt every layer it cut through.

Alessio’s voice reached me just before I went under, low, barely a murmur.

“Hang in there, Gianna. When you wake up, that stage is all yours.”

From whatever sliver of me was still conscious, I answered the voice.

[If this body dies, I go back to my real world. My heart gets fixed. And I get the hundred million dollars, right?]

[Yes.]

What a joke.

I was never supposed to be here.

The only reason I’d agreed to enter this world was desperation. I couldn’t afford my heart surgery. So I took the deal. Come here, save Alessio, Matteo, Dario, and Rafe from the destruction waiting for them.

Fifteen years I’d spent saving them. In return, they gutted me to save her.

I ripped the oxygen cannula out of my nose.

My airway clamped shut. Every breath was a fight my body was losing. My new, ruined heart couldn’t keep up. My lungs burned, and no air got through no matter how hard I tried.

My face went purple. I was seconds from the end.

But I was smiling.

Dying was fine by me. Better than staying here, choking on their lies.

The heart monitor flatlined, a single screaming tone, and red lights started flashing. Alessio, Matteo, Dario, and Rafe burst through the door with a team of nurses right behind them.

All four of them, shouting at once. “Nina!”

Alessio did compressions until my heart started again. He forced the oxygen mask back over my face, and when he looked at me, his eyes were shot through with red.

“What the hell is wrong with you? Are you out of your mind? You ripped out your own oxygen supply. It was a miscarriage, Nina. It’s not worth dying over!”

I pressed my hand against the massive bandage on my chest, the pain blinding, and forced the words out raw.

“Is the baby… the only thing I lost?”
