Rafe stared at me, guilt eating him alive.

“This is on me, Nina. I’m the one who told Gianna to bring you those vitamins.”

“I thought they’d protect the baby. I didn’t know you’d react like that. I didn’t know it would…” His voice broke. “God, the toxins went straight to your heart.”

“But Alessio’s the best cardiac surgeon in the country. He’s going to fix this. And we’ll have more kids, Nina. I promise you that.”

He came to the bed, eyes swollen and red, and took my hand in both of his.

“The doctor who filled that prescription? Handled. I had her license pulled and ran her out of the city. She’s dead to the medical world. Don’t blame Gianna. Don’t blame yourself. You’ve got people who love you, Nina.”

I didn’t answer. Matteo cleared his throat, his voice rough.

“He’s right, Nina. We’ve already reached out to every transplant program we can find, domestic, international, everything. We’re going to buy you a new heart if we have to.”

Dario squeezed my shoulder, his touch gentle, like I was made of glass.

“Don’t worry, princess. I’m going to develop something just for you, a drug that’ll have you back to your old self in no time. Running the estate, all of it.”

All I felt was cold.

The sheer audacity of it all was sickening.

A few pretty words, and they thought they could cover up everything? Gianna had poisoned me, killed my baby, and stolen my heart.

I looked at the four of them standing around my bed, and a bitter smile pulled at my mouth.

“Where’s Gianna? I want to hear it from her.”

Alessio’s jaw tightened.

“You’re seriously still blaming Gianna? The guilt hit her so hard she had a massive heart attack. They barely found a donor heart in time to save her.”

Dario jumped in before I could push back.

“Just focus on getting better, okay? If Alessio could save Gianna, he can fix you too. It’s just a matter of time.”

I let out a harsh, breathless scoff.

“Funny coincidence. My heart fails on the exact same operating table, and Gianna just happens to find a donor right down the hall?”

The room went dead quiet.

“Nina…” Alessio started, then stopped himself, shifting uneasily under my gaze.

I was choking on their bullshit.

I forced a thin, dead-eyed smile.

“Relax, I’m kidding. Go check on Gianna. I’m thrilled she’s doing so well.”

The three of them exchanged uneasy glances, mouths half-open like they wanted to say more.

I turned my back to them, presenting nothing but cold silence.

When I finally looked back, they were gone.

Only Rafe was still there, watching me with that wrecked look on his face.

I smiled at him.

“Go check on her for me. Come back and tell me all about her miraculous recovery.”

He didn’t hesitate this time.

He left.

Finally, I was alone.

I let out a long, shaking breath and unlocked my phone. My hands wouldn’t stop trembling.

The system had made me a deal. Complete the mission, and it would grant me one wish.

I’d cashed in that wish right before they wheeled me into surgery. Two videos. That was what I’d asked for.

I hit play. Then I played them again. And again. I couldn’t stop.

I watched them plan the whole thing, calm and clear, deciding to butcher me and give my heart away.

I watched Alessio cut my heart from my chest and place it, steady-handed, into Gianna’s chest.

Every drop of blood in me turned to ice.

And just like that, I was glad they’d brought me back.

Dying would’ve been too good for them.

I opened a burner account and sent the videos to him.

For the rest of my stay, they took turns sitting with me, one at a time, never for long. They always had somewhere else to be.

I tried to convince myself they were handling family business.

Then one afternoon, I needed a nurse and no one was answering the call button. I grabbed my IV pole and dragged myself out of the room and down the hall.

I passed a VIP suite and glanced through the cracked door.

Rafe was inside. Rafe, who had told me he was rushing off to a meeting. He was carefully slicing a filet mignon and feeding it to Gianna.

Alessio, Matteo, and Dario were all there too, the same men who were supposedly too busy to stay an hour with me. They hovered around Gianna like she was the only person in the world.

Alessio’s voice was all fond exasperation.

“Gianna, you can’t just eat whatever you want because you’re feeling better. Steak isn’t exactly post-op food.”

Gianna tugged at the sleeve of his coat, flashing him a puppy-dog pout.

“It’s not my fault Matteo’s steak is that good.”

Then she groaned, throwing her head back dramatically.

“You guys are the worst, you know that? Hovering over me twenty-four-seven. I can’t even sneak a bite without someone catching me.”

My nails bit into my palms. Something bitter pulled at the corners of my mouth.

In all the days I’d spent in this hospital, not one of them had ever brought me a real meal.

Gianna had all four of them at her bedside every single day, and Matteo, the ruthless Don himself, cooked for her.

I got whatever the aide grabbed from the cafeteria downstairs. Once, the milk was expired.

I turned away.

“Apparently the girl I’m looking after is their biological daughter, the real one. Can you believe that? Dr. Alessio told me to pump her full of IV fluids. They’re loading her up so they can use her as a blood bank.”

I froze near the charting station as two nurses whispered in hushed tones.

“If Gianna’s body rejects the heart, they’re gonna drain that girl dry. A complete blood exchange for Gianna, every last drop.”

Every muscle in my body locked up.
